CERTIFIED COPIES OF BIRTH, DEATH
AND MARRIAGE RECORDS

The North Kingstown Town Clerk’s Office maintains copies of birth records for births, which occurred in North Kingstown or for residents who were living in North Kingstown at the time of their birth. An applicant may request a full size or wallet size certified copy of his or her own birth certificate or that of an immediate family member. The fee is $20.00 for the first certified copy and $15.00 for each additional copy.

You may obtain a certified copy of a birth certificate by mail or in person at the North Kingstown Town Hall, 80 Boston Neck Road, North Kingstown, Rhode Island 02852. If you apply by mail, your request must be accompanied by copy of a driver's license;  a business size self-addressed stamped envelope; and a check made payable to the Town of North Kingstown.  Please also provide a daytime phone number or an e-mail address.   If you need further information on applying in person or by mail, please call the Town Clerk’s Office at 401-294-3331, ext. 122.

Application for a Certified Copy of a Birth Record

DEATH

The North Kingstown Town Clerk’s Office only issues certified copies of death certificates of individuals who died within North Kingstown or who were residents of North Kingstown at the time of their death. Certified copies of death certificates will only be issued to next of kin or to persons with a legal right to the certificate.

MARRIAGE

The North Kingstown Town Clerk’s Office maintains copies of marriage licenses of marriages that occurred in North Kingstown or marriages of residents of North Kingstown. Certified copies of marriage licenses will only be issued to the parties to the marriage or persons with a legal right to the record.

Application for a Certified Copy of a Marriage Record

PLEASE NOTE
Section 23-3-28 of the General Laws of the state of Rhode Island states:

"Any person who willfully and knowingly makes any false statement in a report, record, certificate or application for an amendment thereof, or who willfully and knowingly supplies false information intending that such information be used in preparation of any of the such report, record or certificate, or amendment thereof.... shall be punished (if convicted) by a fine of not more than one thousand dollars ($1,000) or imprisoned not more than one (2) year or both".
